Is every enlargement of Prostate gland due to cancer or something else? Difference in BPH vs Prostate Cancer?

  Enlargement of prostate gland can be due to 1. Prostate Cancer and 2. BPH.

Prostate Cancer-

  • What is Prostate Cancer? Cancer is abnormal growth and uncontrolled growth of any cell in the body that leads to development of swelling or tumor. When this happen in the cells of prostate then it called prostate cancer. In case of cancer, single of multiple, small tumors start growing inside prostate gland. These cells are continuously dividing and growing. This causes development of many swelling / tumor inside prostate that looks like irregular enlargement of prostate.
  • Is it life threatening? Sometime, these cells escape from prostate and spreads throughout body via blood or lymph and make many tumor in other organs of body like lung, liver, bone or brain. When these cancer are confined inside prostate is called localized stage of prostate cancer, when it spread up to nearby regional lymph nodes then called Node positive Prostate Cancer or when spread to other organs like lung then called Metastatic Prostate Cancer or Stage IV prostate Cancer. then it become life threatening.
  • Symptoms of Prostate Cancer: In localized stage, symptoms of prostate cancer are related to obstruction in urethra due to tumor like difficulty in passing urine, increased frequency of passage of urine. incomplete evacuation of urine, blood in urine etc. In advanced stage, it become life threatening due in involvement of vital organs like liver, lung, brain and organs etc.
  • Treatment of prostate Cancer: Its treatment depends on histopathology or biopsy of prostate cancer, serum PSA level and stage of prostate cancer. Common form of prostate cancer are Adenocarcinoma (Mainly Acinar or ductal) while neuroendocrine or small cell prostate cancer are rare but aggressive variants.
  • Unlike other cancer, it is mainly treated by radiotherapy and Hormonal treatment or surgery. Rarely, it require chemotherapy like in Small cell or high grade neuroendocrine pathology.



Difference in BPH and Prostate Cancer?


BPH- It is called benign Prostate Hyperplasia.

  • What is BPH? As, we grow and become older, we see a lot of changes in our body and hormonal system. Benign prostatic hyperplasia is one of these changes where cells of cells present in central (transitional Zone) of prostate grows in number and size that cause obstruction in passage of urine and produces symptoms like decreased Urinary flow or dribbling of urine. Sometime, man not able to pass urine due to complete obstruction (retention of urine in bladder).
  • Symptoms of BPH: Symptoms of BPH are collectively called LUTS (Lower Urinary tract symptoms) like increased urinary frequency, weak stream of urine, dribbling of urine, unable to pass urine, increased frequency of passage of urine, increased tendency to pass urine at day time as well as in night (nocturia), urgency to pass urine.
  1. Is it life threatening? This problem is neither life threatening nor spread. But its treatment is simple and essential. As, chronic obstruction in passage of urine leads to swelling of ureter (Hydronephrosis) and Chronic Kidney Damage (CKD).
  2. Treatment of BPH: Its treatment is simple, mostly done with medication. Sometime, if urinary retension then one may require catheterization or widening of passage of urine (Urethra) by removing chips of prostatic tissue with help of laser. This minor surgical procedure is called TURP (Transurethral Resection of Prostate).
